The RN Dialysis Apheresis a professional caregiver, assumes responsibility, accountability for assigned patient group for a designated time frame and during dialysis apheresis procedure. Provides care to patients via therapeutic use of the nursing process, environment, treatment and modalities and interdisciplinary collaboration. Coordinates, delegates and provides direct nursing care for patients by working cooperatively with patients, family members and patient care team members.

Job Description

GENERAL DUTIES

* Assessment/Reassessment and Implementation: * Conducts a comprehensive physiological and psychosocial assessment upon admission or upon assuming patient care. * Completes focused reassessments according to hospital policy. * Documents intervention implementation, noting exceptions/modifications and plan of care in the medical record. * Implements the interventions identified in the plan of care in a safe and timely manner such as administering prescribed medications or carrying out procedures. Prioritizes the implementation of interventions based on patient's condition. * Notifies the physician of changes in the patient's condition.

* Quality and Clinical/Technical Expertise: * Implements infection control practices. * Maintains the environment of care consistent with Joint Commission requirements. * Participates in quality improvement activities and review/audit processes. * Consistently administers medications following the hospital's policies to ensure safe medication administration and documents patient's responses to medications

* Communication and Staffing: * Maintains communication with other caregiving staff and performs proper hand-off utilizing SBAR format to ensure continuity of patient care.

* Education and Planning: * Educates patient and caregiver on the plan of care as needed. * Reviews general hospital and environmental policies with the patient and caregiver upon admission. * Educates patients and caregivers regarding HAC and infection control practices and about medications and medical equipment. * Documents all education provided to the patient/families. * Identifies patient-specific needs and problems. Collaborates with the healthcare team to develop an individualized plan to achieve desired patient outcomes.

JOB SPECIFICATIONS:

Education:

Minimum Required: Graduate of an accredited professional school of nursing. Enrollment in a BSN program with in the first year of hire and completion within 3 years.

Preferred: BSN

Experience:

Minimum Required: Sufficient previous clinical experience in area of expertise.

Minimum Required: Current Louisiana Nursing License. CPR verification. Meets specific departmental criteria and/or certifications.

FUNCTIONAL DEMANDS
