Our more than 30 programs throughout the district provide comprehensive services that lead to mental and physical health, continued sobriety, job training, living-wage employment, and affordable housing. Compensation:
We offer our employees a competitive compensation and benefits package that reflects our organizational culture, mission, and core values. The salary range for this position is $78,644 to $83,199 and may commensurate with experience.
SOME (So Others Might Eat) is currently seeking a full-time
Clinical Social Work Program Manager
for our Senior Services department.
Position Description:
The Clinical Social Work Program Manager provides program and administrative oversight of Kuehner Place, SOME's 7-bed transitional housing program for older adults experiencing homelessness. Manages intake, enrollment, and intensive case management processes for prospective and current program participants. Participates as an active member of the clinical care team and provides clinical support and consultation to Kuehner House and Karin House residents and staff. Independently supervises night and front desk monitors, and clinical interns.
Schedule:
Monday-Friday; 8:00 am-4:30 pm (40 hours a week)
Required:
Master of Social Work (MSW) and be licensed in the District (LGSW or LICSW) with 2+ years of previous case management experience with senior and/or at-risk populations.
Preferred:
Master’s Degree in Social Work; 5-7 years experience providing services to seniors and low-income populations; 2-3 years of case management experience; previous experience providing services in an interdisciplinary care team.
Required License/Certification:
CPR/First Aid, AED, Driver's License, LGSW, or LICSW by the District of Columbia.

Program Management:
Oversee management of waitlist and program intake. Assist with social service record collection for new residents ensuring efficiency and compliance with program guidelines protocol and compliance. Collaborate with the property management team to ensure timely move-ins and discharges Obtain program supplies, including food and necessities, for residents of Kuehner Place based on program goals and budget Identify, track, and accurately report program statistics Develop administrative report program statistics Ensure compliance with guidelines of funding sources and applicable governing bodies to provide case management. Create and maintain client records including documentation, case notes, assessments, etc. Communicate regularly with evening and weekend monitor staff to identify client needs and progress. Contribute to department budget development, and use sound judgment to leverage resources in accomplishing department goals; Administer the budget and comply with SOME financial reporting requirements Contribute to strategy development and strive to improve department functions and performance; Assist in the development of standards and goals as appropriate. Engage in and deploy an emergency response plan, as needed.
Case Management:
Assess senior clients and define barriers Provide resource referrals in the form of one-on-one support sessions, family meetings, and group sessions Develop person-centered care management plans with clients Represent Kuehner Place as a member of the clinical care team Meet with the Clinical Director and care team for client planning collaboration and guidance Implement care interventions with clients Partner with medical clinic staff to ensure the management of resident chronic health diagnosis Develop case notes and provide status updates Escort and assist residents to medical, housing, and other necessary appointments Assist residents in identifying and obtaining permanent housing Collect and appropriately secure funds from residents to be held in escrow for housing Participate in senior services care team including meetings, case conferences, and training Manage resident issues and provide mediation and coaching Support Kuehner House staff with KH crisis as needed.
Supervisory:
Ensure appropriate department staffing levels in order to meet objectives, Assess, Evaluate, and hire new employees as needed. Submit supervisory and administrative paperwork to Human Resources and Finance, as required; Refer to SOME Employee Handbook for policy guidance. Maintain professional documentation and ensure the program compliance with SOME CQI Standards Ensure compliance with all state, local, and federal laws governing the workplace and all department functions. Schedule staff work hours as needed, and monitor for time and attendance; Approve time cards and requests for leave. Provide direction and guidance to staff members on department operations and procedures Define work requirements, assign tasks to staff members, and review work for quality and accuracy; Coordinate with Human Resources to update job descriptions as needed. Provide regular feedback on performance, monitor individual performance, conduct performance evaluations, and take steps for performance improvement as needed. Provide coaching and training for staff, assist staff with work processes and policies, and encourage opportunities for professional development.
