Pathways to Housing DC (Pathways DC) is based in Washington, DC and serves people in the District and Montgomery County, MD. We are an innovative and nationally recognized nonprofit committed to ending homelessness for individuals with complex health challenges. Pathways DC is one of the originators of the "Housing First" model to end homelessness, in which services are provided without conditions and pre-housing requirements such as curfews, mandated sobriety or compliance with medication. We believe housing is healthcare and that housing is a basic human right. Pathways provides an array of services including housing, street outreach, case management, drop-in services, and behavioral health support. Our model is based on client choice and the idea that the people we serve are the architects of their recovery!The Permanent Supportive Housing Team Leader is responsible for the daily operation and clinical supervision of Case Managers of the Permanent Supportive Housing Program (PSHP). The Team Leader is responsible for assuring that the agency values and philosophy of consumer choice, harm reduction and respectful concern are expressed in all aspects of team practice. The Team Leader maintains oversight to ensure that client self-identified goals for recovery, rehabilitation and community integration are supported by appropriate services and best practices.

RESPONSIBILITIES:

Supervise and facilitate the professional development of the Case ManagersCollaborate with the Program Manager in determining staffing needs and interviewing potential Case Managers, as well as assigning caseloads to Case ManagersTrain, supervise and evaluate the Case Managers in conjunction with the Clinical Supervisors and Program ManagerPlan and implement all client services, in collaboration with the Case ManagersSupervise electronic and written record-keeping activities, e.g., progress notes, service plans, client budgets, incident reports and client chartsCoordinate services of the PSHP with other inpatient and outpatient community treatment, support and social servicesCollaborate with the Program Manager regarding housing issues of PSHP clientsRepresent PSHP in internal meetings. May represent Pathways on external related meeting in conjunction with the Clinical Supervisors and Program ManagerCoordinate the use of agency vehicles designated for use by the PSHPPerform other related duties as assignedQUALIFICATIONS:

Master's Degree in Social Work, Mental Health Counseling, or related field from an accredited College.DC licensure as a Licensed Independent Clinical Social Worker (LICSW), Licensed Graduate Social Worker (LGSW) or a Licensed Professional Counselor (LPC).2 years' experience in case management and/or counseling services to individuals/families experiencing homelessness or other related populations.Experience supervising staff performing case management services strongly desired.Excellent interpersonal, communication and writing skills, as well as computer literacy.Patience, creativity, flexibility, compassion, and sensitivity to persons with disabilities and other minority populations.Bilingual English/Spanish strongly desired.Driver's license and access to motor vehicle required
