Job Description:

• Seeking a compassionate and experienced RN Case Manager to join our dedicated team in The Woodlands, Texas.

• As an RN Case Manager, you will play a crucial role in providing high-quality hospice and palliative care to our patients, ensuring they receive the best possible support during their end-of-life journey.

• Conduct comprehensive assessments of patients' needs, symptoms, and conditions

• Develop and implement individualized care plans in collaboration with the interdisciplinary team

• Provide skilled nursing care, including pain and symptom management, wound care, and medication administration

• Educate patients and families about their conditions, treatment options, and care plans

• Coordinate with physicians, social workers, and other healthcare professionals to ensure holistic and patient-centered care

• Monitor and document patients' progress, adjusting care plans as necessary

• Advocate for patients' and families' needs, ensuring they have access to necessary resources and support

• Participate in team meetings and case conferences to discuss patient care and outcomes

Qualifications:

• Current RN license in the state of Texas

• Minimum of 2 years of experience in hospice, palliative care, or a related field

• Strong clinical skills and knowledge of end-of-life care practices

• Excellent communication and interpersonal skills

• Ability to work independently and as part of an interdisciplinary team

• Compassionate and empathetic approach to patient care

• Valid driver's license and reliable transportation
