Job Description:
Seeking a compassionate and experienced RN Case Manager to join our dedicated team in The Woodlands, Texas. As an RN Case Manager, you will play a crucial role in providing high-quality hospice and palliative care to our patients, ensuring they receive the best possible support during their end-of-life journey. Conduct comprehensive assessments of patients' needs, symptoms, and conditions Develop and implement individualized care plans in collaboration with the interdisciplinary team Provide skilled nursing care, including pain and symptom management, wound care, and medication administration Educate patients and families about their conditions, treatment options, and care plans Coordinate with physicians, social workers, and other healthcare professionals to ensure holistic and patient-centered care Monitor and document patients' progress, adjusting care plans as necessary Advocate for patients' and families' needs, ensuring they have access to necessary resources and support Participate in team meetings and case conferences to discuss patient care and outcomes Qualifications:
Current RN license in the state of Texas Minimum of 2 years of experience in hospice, palliative care, or a related field Strong clinical skills and knowledge of end-of-life care practices Excellent communication and interpersonal skills Ability to work independently and as part of an interdisciplinary team Compassionate and empathetic approach to patient care Valid driver's license and reliable transportation Benefits:
