Job Description
Exhibits skills needed to work in the clinical pharmacy program with emphasis on Psychotropic medication usage, drug interventions and food and drug interactions with respect to disease state management.
Demonstrates ability to conduct chart reviews, review medication errors, adverse drug reactions, compliance to protocols and monitoring guidelines as it relates to Psychotropic agents.
If needed, help in unit dose dispensing.
Supervises a Pharmacy unit and/or performs specialty Psychotherapeutic clinical pharmacist duty in the Psychiatry setting.
Interacts with other staff members, patients and visitors, serves on interdisciplinary teams and provides education to health care providers, patients and their families.
Monitoring of specific medication therapy and optimize the use of psychotropic medications, while decreasing the risk for adverse reactions and drug-drug or drug food interactions.
Publishing Pharmacy Newsletter.
Providing orientation to health care providers and nurses.
In-service Physicians/Nurses/Pharmacists as needed for new/existing Psychotherapeutic agents and/or disease state management.
Presenting during Psychiatric Grand Rounds as needed.
Answers drug information questions.
Compilation of monthly Interventions.
Follow-up of significant ADR's.
Organize and manage group and individual medication education sessions for patients and family on the Psychiatry units.
Organize statistical data as needed for the P&T Committee.
Develops and implements new processes to improve the operation of the Pharmacy and the quality of patient care provided.
Ensures staff compliance with regulatory agency and hospital rules and regulations.
Evaluates new drugs released by the FDA in terms of efficacy, adverse effect profile, and cost; and makes recommendations on additions and deletions to the facility's Psychotropic category of the drug formulary.
Manages medication therapy of a group or unit of patients as part of an interdisciplinary team.
Coordinates medical and psychiatric disease state management to ensure proper use of medications in patients with concurrent medical conditions.
Serves as a professional and technical resource for medical professionals in Psychiatric units.
Pharmacologic and non-pharmacologic Psychiatric treatment planning for inpatient and outpatient population.
Design and implement monitoring plan to ensure the safety of Psychotropic's in vulnerable populations.
Other duties as deemed relevant for provision of patient care
Minimum Qualifications
For Level I: 1. Possession of a valid NY State license to practice pharmacy.
For Level II:
In addition to meeting the qualification requirements of the lower level: 1. Two years of post-licensure experience; or 2. Completion of a post graduate specialty residency or fellowship program; or 3. Current Pharmacy Board Certification.
For Level III:
In addition to meeting the qualification requirements of the lower levels, an additional: 1. One year of post licensure supervisory experience or specialty clinical experience.
