Hours for this role are Monday - Friday, 8a-430p The Intensive Outpatient Program (IOP)/Partial Hospitalization Program (PHP) Social Worker provides services within the IOP or PHP service line for individuals with psychiatric needs. Services may be offered in person or remote based on patient need, though the Social Worker is based out of our locations, in person. The Social Worker focuses on facilitating admissions, psychoeducation groups, treatment planning, discharge planning, care coordination, communication with the patient's support system, community providers, and others involved in the care of patients. Social Workers collaborate daily with psychiatric medical providers and therapists on the patient's primary Treatment Team to advance the plan of care within the average patient length of stay. Responsibilities and Accountabilities: Accountable to uphold and apply knowledge and understanding of emotional and behavioral health diagnosis and treatment Responsible for effective psychosocial assessment and suicide risk assessment, discharge planning, and leading psychoeducational groups Develop and revise treatment and complex discharge plans and communicate these with patients, their support system, and the treatment team Responsible for communicating with patients and their support system to keep them involved in treatment plan decisions Provide psychoeducation to patients, their support system, and the treatment team, regarding admission process, diagnosis, symptom management, recommendations, discharge plans, safety plans, patient rights, financial impact issues, and other pertinent patient care matters Be oriented to ethically provide and advocate for patient-centered, culturally appropriate, age-appropriate care Maintains current competency in effective de-escalation of patients using Collaborative Problem Solving , therapeutic self, relationship-based care, and trauma informed care, as applicable to role. Qualifications
Bachelor's Degree in Social Work required, Master's Degree in Social Work preferred, with licensure in the State of Minnesota required. Experience in a mental health setting preferred Master's Degree in Social Work preferred, with licensure in the State of Minnesota required. Experience using electronic health records preferred Proficient in Microsoft Office Products including Word Outlook, and Teams Able to effectively and efficiently communicate in written and verbal form Ability to smoothly and rapidly shift between dissimilar tasks Ability to communicate well both verbally and in writing Ability to respond appropriately in times of mental health crisis Pay range for this position is $54,080 to $75,795 depending on education and experience.
