The Community Health Centers

We are currently seeking a Community Health Worker to join our team as we expand our clinics to offer accessible care in our communities.
The Community Health Worker (CHW) will be embedded into Community Health Center (CHC) patient care teams to provide screening, follow-up and navigation to services for mental health and substance use disorders, and social determinants of health (SDOH). The CHW will facilitate access to services with a goal of improving patient health and well-being. The CHW will employ population health metrics to identify patients in need of services.
Basic Qualifications:
Education and Experience
High school diploma or equivalent Previous experience providing social services, translation services, case management services, or other client or customer services. Experience with health coaching a plus Knowledge, Skills, and Abilities
Bi-lingual speaker/writer preferred. Strong written and verbal communication skills. Ability to complete all written paperwork in a timely manner is essential (written care plans, progress notes, etc.) Good organizational and problem-solving skills. Ability to seek supervision when needed. Ability to work with diverse populations. Strong boundaries and ethical decision-making. Willingness to work flexible hours, including evenings if necessary. Ability to work as part of multi-disciplinary team. Licensure, Certification, or Other Requirements
Must have access to transportation.
