Company Overview and Mission
Columbus House opened its doors in 1982 to provide services for men and women at least 18 years of age. Our goals quickly broadened from the mere provisionof food and shelter and simple survival, to understanding and working toward overcoming the problems which cause people to become homeless.
Columbus House, along with its core of loyal supporters, friends, and volunteers, has remained committed to fulfilling its mission: "To serve people experiencing homelessness or at imminent risk by providing life-saving outreach, shelter and housing and by fostering their personal growth and independence. We advocate for and create affordable housing to end homelessness in our communities."
Purpose
To provide case management to homeless shelter residents; to network with the greater New Haven shelter system and mental health/substance abuse/social/medical/housing service systems; to collaborate with clinicians and other service providers as well as other Columbus House Case Managers; to manage, link to services, support housing search; to empower residents.
Duties and Responsibilities
Case manages 25 shelter residents.
Provide front door assessments and diversion as part of the Coordinated Access Network.
Service and engage consumers weekly at minimum or as needed, building a trusting working relationship.
When appropriate, make referrals to agencies for SA/MH, medical, vocational, educational, or housing needs.
Link consumers with entitlement providers as needed.
Occasionally provide transportation to medical/clinical appointments using agency provided veh
