Created in 1993, LAHSA is a joint powers authority of the city and county of Los Angeles. As the lead agency in the HUD-funded Los Angeles Continuum of Care, we coordinate and manage over $800 million annually in federal, state, county, and city funds for programs providing shelter, housing, and services to people experiencing homelessness. The Outreach Navigators (CR) works as a team member of the Resource and Referral team, in collaboration with social service professionals, ensuring the delivery of quality care in accordance with the rules and regulations established by the Los Angeles Department of Health Services for contracted agencies. The Outreach Navigators will do all that is necessary to help people experiencing homelessness get connected to resources, supportive services, and housing. In this role, Outreach Navigator will engage unhoused sleepers, respond to LAHOP and emergency requests, assist with encampment repopulation management, and support overnight operations at Interim Housing and/or Motels within the county, as needed. Shift may require you to work weekends and holidays. Working hours will be from 5pm-1:30am but may be required to support other shifts as needed. The functions listed below are intended to describe the general nature and level of work being performed and are not to be interpreted as an exhaustive list of responsibilities. Provide street-based outreach and supportive services to individuals experiencing homelessness. Provide support and assistance to clients with accessing resources in the community. Establish and maintain professional and positive relationships with staff, clients, law enforcement, and community providers. Adhere to housing first and harm reduction models of advocacy with clients. Conduct interactions with clients and other professionals in a respectful and courteous manner. Listen attentively to client needs and suggestions to address their issues fairly and professionally. Coordinate with immediate supervisor for supervision. Conduct CES Assessment to determine appropriate housing intervention and service needs. Link clients to appropriate bridge/interim housing programs. Assist clients in completing required documentation to enroll into permanent housing-related programs. Transport clients to gather necessary paperwork for housing, including but not limited to the DMV, Social Security Office, and Department of Social Services. Assist clients with accessing resources and services to increase housing stability (e.g. linkage to primary care physicians, health insurance, mental health/substance abuse support, benefits advocacy, employment services, food banks, credit repair, legal aid, In Home care). Support Welcome Navigation Center, and other emergency response programs, as needed. Perform other duties and special projects, as assigned. Minimum Requirements: Must have excellent organizational skills and the capability to work in a fast-paced environment. In emergencies must be willing to work when necessary. Understand how to work with low income and homeless individuals. Knowledge of how to work effectively in a harm reduction environment. Always demonstrate cooperative and professional behavior and maintain a professional appearance. Strong knowledge of homeless services and resources. Demonstrated knowledge and experience with Harm Reduction, Motivational Interviewing, Critical Time Intervention, and Housing First. Effective crisis management skills. Knowledge of the LA CoC Homeless Management Information System (HMIS). Maintain up to date Clarity (HMIS) Records for services provided to clients directly in the field. Excellent people skills, specifically customer service skills and a capacity for collaboration and interpersonal relationships. Excellent written and verbal communication skills, including the ability to express technical concepts clearly to both technical and non-technical audiences. Strong organizational skills with a strong attention to detail. Intermediate level of computer software programs specifically, Microsoft Office (Word, Excel, Access, Power Point and Outlook). Ability to simultaneously manage multiple projects and timelines. The ability to perform the physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. This is a very active position. While performing the duties of this job, the employee is regularly required to walk, climb or balance; and stoop, kneel, crouch or crawl. The employee must occasionally lift and/or move items over 10 pounds. Additional Requirements: Proven professional experience and educational background. A valid California Class C Driver License is required to carry out job-related essential functions and a good driving record for a minimum of three (3) years. Schedule: The Chief Executive Officer or designee may change the schedule of positions between hybrid and onsite, as necessary to accomplish LAHSA's business needs.
