Job Description
Areas of Responsibilities:
Nutrition Office, Kitchen, Inpatient and Behavioral Health Units
Summary of Duties and Responsibilities: 1. Provides nutrition services for the patient's nutritional and metabolic status and nutrient needs 2. Reviews clinical and biochemical data to formulate a nutrition care plan 3. Develops and implements MNT protocols, which include a nutritional assessment based on diet history, clinical data and educational needs 4. Documents care plans, consultations, recommendations and follow-up interventions in the electronic medical record 5. Participates in multidisciplinary team meetings. 6. Collaborates with other institutions/organizations on nutrition-related projects and courses 7. Participates in various community-based outreach programs 8. Instructs nutrition classes and lectures as needed to patients, employees, students and clinical staff; resident physician education lectures 9. Assists in the development of culturally sensitive nutrition educational material to meet the needs of an ethnically diverse patient population 11. Maintains continuing education in nutrition, clinical dietetics and related areas as appropriate in order to maintain current knowledge and skills in the practice of clinical nutrition 12. Documents in Electronic Medical Record (EMR) in the ADIME format utilizing the Nutrition Care Process. 13. Provides medical nutrition therapy (MNT), including discharge instructions, to inpatients and/or families and document nutrition intervention in EMR. 14. Lead Performance Improvement projects, Quality Academy and complex PI development (such as volume-based feeding malnutrition coding and billing) 15. Participates in data collection for performance improvement. Performs and maintains performance improvement programs. 16. Visits patients during meal periods and observe patient diet tolerance, intake and initiate corrections of diet as/if needed. Conducts patients' tray evaluations. 17. Maintains credentialing as a registered dietitian 18. Performs related duties as required
Minimum Qualifications
Qualification Requirements:
For appointment to Assignment Level I 1. Baccalaureate degree from an accredited college or university, with a major in Dietetics or Foods and Nutrition in a program approved by the American Dietetic Association (ADA); or, 2. Registration or eligibility for registration by the Commission on Dietetic Registration (CDR) as a Registered Dietitian or New York State Certification as a Dietitian-Nutritionist.
For appointment to Assignment Level II 1. Registration or eligibility for registration by the CDR as a Registered Dietitian or New York State Certification as a Dietitian-Nutritionist.
For appointment to Assignment Levels III and IV 1. Registration by the CDR as a Registered Dietitian or New York State Certification as a Dietitian-Nutritionist.
Special Note for Dietitians assigned to Post-Acute Care (Long-Term Care) facility: Under the Medicare and Medicaid Programs, Reform of Requirements for Long-Term Care Facilities, as outlined in Section 483.60 of Title 42 of the Code of Federal Regulations all newly-hired Dietitians, Levels I-IV assigned to a Post-Acute Care (Long-Term Care) facility, are required to have New York State Certification, issued by the New York State Education Department (NYSED), as a Dietitian-Nutritionist at the time of hire. Dietitians, Levels I-IV hired prior to November 28, 2016, assigned to a Post-Acute Care (Long-Term Care) facility are required to obtain New York State Certification, issued by the NYSED, as a Dietitian-Nutritionist on or before November 28, 2021.
