Job Description

Purpose:

Under the direction of the coordinating manager and in coordination with the weight management / bariatric physician, provides medical nutrition therapy to adult patients in the weight management / bariatric practices. The dietitian follows hospital and departmental policies and procedures and participates in all interdisciplinary activities as a member of the healthcare team.

Specific Duties and Responsibilities:

The dietitian level IV is charged with and responsible for:

Provides nutrition services for facility-based weight management / bariatric programDocuments care plans, consultations, recommendations and follow-up interventions in the electronic medical recordAssists in the development and implementation of medical nutritional therapy protocols which include our nutritional assessment based in diet history, clinical data, and educational and social needsReviews clinical and laboratory data to formulate a weight management care planEducates and counsels patients on the nutritional management of chronic diseases with an emphasis on obesity, pre-diabetes, and hypertensionProvides extensive education on diet using motivational interviewing techniquesConsult with other team members as necessary, including physicians and clinical psychologistParticipates in hospital wide health fairs, staff health challenges, and community-based outreach programsCreates culturally sensitive educational materials and facilitates educational groups, shared medical appointments for program participants with attention to literacy levels and diverse patient population needsOffers nutrition classes, cooking demonstrations, and lectures as neededParticipates in the development and implementation of quality improvement activities as assignedReports problems to immediate supervisor and recommends ways to improve nutritional servicesPerforms related duties as required

Minimum Qualifications

Qualification Requirements:

For appointment to Assignment Level I1. Baccalaureate degree from an accredited college or university, with a major in Dietetics or Foods and Nutrition in a program approved by the American Dietetic Association (ADA); or,2. Registration or eligibility for registration by the Commission on Dietetic Registration (CDR) as a Registered Dietitian or New York State Certification as a Dietitian-Nutritionist.

For appointment to Assignment Level II1. Registration or eligibility for registration by the CDR as a Registered Dietitian or New York State Certification as a Dietitian-Nutritionist.

For appointment to Assignment Levels III and IV1. Registration by the CDR as a Registered Dietitian or New York State Certification as a Dietitian-Nutritionist.

Special Note for Dietitians assigned to Post-Acute Care (Long-Term Care) facility:Under the Medicare and Medicaid Programs, Reform of Requirements for Long-Term Care Facilities, as outlined in Section 483.60 of Title 42 of the Code of Federal Regulations all newly-hired Dietitians, Levels I-IV assigned to a Post-Acute Care (Long-Term Care) facility, are required to have New York State Certification, issued by the New York State Education Department (NYSED), as a Dietitian-Nutritionist at the time of hire. Dietitians, Levels I-IV hired prior to November 28, 2016, assigned to a Post-Acute Care (Long-Term Care) facility are required to obtain New York State Certification, issued by the NYSED, as a Dietitian-Nutritionist on or before November 28, 2021.
