JOB SUMMARY

The Integrated Outpatient Behavioral Health Clinician (BHC) is part of a multidisciplinary Integrated Care Team that consists of Primary Care Physicians, Registered Nurses (RN), Navigators, Case Managers, Clinical Lead and other specialty providers. The Integrated Outpatient Behavioral Health Clinician is responsible for ensuring that each patient is provided the services necessary to achieve the goals as outlined in their individualized treatment plan. They familiarize themselves, through review of the medical/clinical file, with each new patient who is assigned to them, including their assessment and treatment plan, provide counseling services based on best practices models to include group, individual, and family formats as well as provide educational presentations to patients and/or their families as needed. The Integrated Outpatient Behavioral Health Clinician completes comprehensive assessments based on the design of their program, they assess the patient's status through program defined tools and provide clinical recommendations to the treatment team.

MINIMUM EDUCATION & EXPERIENCE REQUIRED

Master's degree in field related to behavioral health and/or has been licensed at the associate level with The Arizona Board of Behavioral Health Examiners as Psychiatrist, Behavioral Health Medical practitioner, Psychologist, Social worker, Counselor, Marriage and Family Therapist, Substance Abuse Counselor or Registered Nurse required.

1-3 years of full-time Health Care related work experience where his or her prior experience would provide adequate exposure to both behavioral and medical crisis situations is preferred but not required.

Or equivalent combination of education and experience.

LICENSES, CERTIFICATIONS, CREDENTIALS, REGISTRATIONS

Current AZ Driver's License

Clean 39-month motor vehicle record

Licensure at the associate or independent level with The Arizona Board of Behavioral Health Examiners as Psychologist, Social worker, Counselor, Marriage and family therapist or Substance Abuse Counselor is preferred.
