OUTREACH CASE MANAGER

The Outreach Case Manager provides support and advocacy to adults with mental illness who are in need of treatment engagement interventions. These individuals may be at risk of hospitalization, homelessness, physical illness and/or other high-risk circumstances. This professional collaborates with psychiatric and allied team members in treatment planning and intervention.
Essential Duties:
- Completes all required trainings and attends internal and external meetings as required.
- Reports to work on time as scheduled and at the location assigned.
- Utilizes the Core Elements of Case Management to ensure quality of care.
- Completes comprehensive assessments incorporating physical, mental, emotional, and dental health, substance use, housing, food and financial insecurities, legal and vocational needs of individuals within required timeframes.
- Assesses and documents co-occurring disorders includes staging and stage-matched interventions.
- Develops and reviews all required LBS, PIHP and MDHHS person-centered planning and related documents (assessments, referrals, entitlement applications, etc.), within required timeframes.
- Progress and Chart notes meet LBS quality standards and timeframe completion requirement within 24 hours of service delivery or next business day.
- Returns client phone messages within 24 hours or the next business day.
- Assumes temporary caseload assignment of individuals disengaged from treatment.
- Provides outreach interventions to individuals who are disengaged from treatment.
- Coordinates and schedules agency and community appointments for mental health treatment, physical healthcare, and other needed resources.
- Coordinates and/or provides transportation to consumer for appointment linkage.
- Performs follow-up outreach efforts to individuals to maximize treatment engagement.
- Maintains a strength-based approach, using patience, empathy and Motivational Interviewing techniques.
- Effectively utilizes a welcoming philosophy and supportive therapeutic interventions reflecting an understanding of the consumer's emotional needs.
- Responsible for assessing treatment barriers and providing supportive engagement interventions and resources.
- When indicated, completes discharge process, including MDHHS required documents, notifications, and discharge summaries within required timeframes on individuals no longer receiving services.
- Completes other duties as assigned.
- Ability to accurately assess the needs of the individual while identifying the most beneficial and appropriate intervention.
- Excellent communication skills, both verbal and written.
- Ability to establish professional working relationships with all staff.
- Appropriately articulates the view of the agency when networking in the community.
- Demonstrates knowledge of Agency and community resources available to assist consumers during crises.
- Works as part of a treatment team to facilitate efficient operation of the program and consumer services.

EDUCATION AND EXPERIENCE
- Bachelor's degree in social work or nursing from an accredited college or university or Master's degree in social work, counseling or psychology from an accredited college or university is required.
- Prefer 2 years of experience in working with the mentally ill population.

- Possession of a valid Michigan Driver's License with an acceptable driving record according to Lincoln Behavioral Services policy, including issuance for a minimum of 3 years.
