Overview

Position:

The Case Manager - CheckPoint (aka Be@School) is responsible to deliver culturally responsive case management by strengthening family/school connections for youth ages 5-17 who are experiencing barriers to attendance, interested in increasing their independent living skills and learning to identify positive healthy relationships.

Qualifications:

  • Bachelor's degree in social work, psychology, sociology, or a closely related field from an accredited four-year college or university.
  • Experience working with public, private and charter schools preferred.
  • Minimum two (2) years of case management experience, or experience delivering youth development/OST programs.
  • Minimum one (1) year working with youth and young adults from various ethnic and cultural backgrounds, ages, as well as economic circumstances.
  • Experience identifying and assessing a wide range of children's needs and working with local community resources.
  • Must have a valid driver's license, reliable transportation and the ability to meet NorthPoint's driving standards.
  • Must be proficient in MS Word, MS Excel, and computer client tracking databases.
