FLORIDA DEPARTMENT OF HEALTH - OPEN COMPETITIVE OPPORTUNITY
This position is a responsible professional position that reports Director of Healthcare Quality and Clinical Management Unit for the Division of Children's Medical Services. This position is responsible for promoting an integrated, comprehensive, and cost-effective health care system with a focus on health equity and social determinants of health to ensure access to quality care for CMS Plan enrollees. This incumbent will serve as the population health subject matter expert and will perform independent and complex work. This role is responsible for coordinating resources, leading implementing projects, and day-to-day coordination, oversight, and monitoring for efficient operation of Population Health programs such as Medical Foster Care (MFC), Prenatal, Early and Periodic Screening, Diagnosis and Treatment Program gram of All-Inclusive Care for Children (PACC)/Partners in Care-Together for Families (PIC:TFK), and Early Intervention Services (EIS) programs. This incumbent will be a Community Health Nursing Consultant for related CMS Plan Program initiatives and activities which include but are not limited to Care Coordination/Case Management and Population Health.
Your Specific Responsibilities:
* Performing monitoring activities that include case reviews, report reviews, leading initiatives for quality improvement projects/developing best practices and analyzing QI data in the area of Population Health. * Analysis and trending of population data which includes the PIC:TFK, MFC, EPSDT, EIS, prenatal population, and all population health programs. * Participating and ensuring CMS Plan vendor representation for all staffings, workgroups, and meetings related to Population Health. Maintain tracking log of staffings and provide consultation on follow ups needed with the CMS Plan vendor. * Leading PIC:TFK workgroup meetings. * Review and maintain quality of care complaints when escalated to the CMS Managed Care Plan Central Office for clinical reasons related to Population Health. * Performing program monitoring (desk and on-site reviews) to ensure that all requirements are being met in relation to the population health program. * Assists the Healthcare Quality and Clinical Management Director with special projects as assigned. * Performs other related duties as assigned.
Required Knowledge, Skills, and Abilities:
* Knowledge of care coordination/case management, population health, managed care, and CMS policies; * Knowledge of the principles and delivery of health services; * Proficient in Microsoft Office and strong computer skills; * Strong communication and presentation skills, training/meeting facilitation skills; * High degree of independence, flexibility, initiative, and commitment; * Knowledge of population principles, practices and techniques; * Ability to lead quality improvement projects and provide technical assistance * Ability to collect, interpret, and analyze statistical, demographic, and service data; * Ability to understand, interpret, and apply applicable rules, regulations, policies and procedures; * Ability to communicate effectively, both written and verbal; * Ability to establish and maintain effective working relationships with others; * Excellent organizational skills and coordination skills, including multitasking and setting priorities on work assignments; * Excellent time management skills; * Excellent writing, research, analytical skills;
Qualifications:
* This position is required to have and maintain a clear and active Registered Nurse License in the State of Florida (F.S. Reference: 464) or Multistate Registered Nurse License from another Compact State. * Years of Experience: 1-3 Years of pediatric clinical experience preferred. * Travel time: 5%.
