Our groundbreaking hospice and palliative care programs offer a broad range of facility and community-based services to help patients and their families ease the burden of managing a life-limiting illness or end-of-life care.

The CSR serves as a liaison between Hospice Home Care Planners, Social Workers, RN Case Managers, and RN on-call Triage Nurses. The responsibilities include:

  1. Responding to Social Work, RN Case Manager, patient, and customer complaints and grievances.
  2. Taking accurate messages and problem-solving where applicable, then forwarding messages to the appropriate party as directed.
  3. Entering patient referral information into the Allscripts system, including patient demographics, diagnosis, and plan of care.
  4. Sending tasks to Team Assistants for Insurance Verification and to vendor services for DME and HHA.
  5. Verifying that all new physicians' license numbers are valid, that NPI numbers are registered, and checking for duplicate physician entries before entering them into Allscripts.
  6. Contacting Physicians, Clinics, Hospitals, Nursing Homes, and sub-acute facilities to develop, maintain, and follow up on pending and/or new referrals.

Minimum Requirements:

  • High school diploma or GED required.
  • Minimum 6 months call center experience.
  • Intake experience preferred.
  • Microsoft Word and Excel required.
  • STAT, Omni/MISYS preferred.
  • Strong verbal communication skills.
  • Ability to multi-task and work in a fast-paced environment.
