This position has a pay rate of $46,500.00 annually

_________________________________________________________________________________ Independent Living Plus (ILP) is a transformative program to increase positive outcomes for youth aging out of foster care by empowering and equipping them for independence. This role leads and coordinates their team of staff to support teens in the program. In addition, the Team Leader for ILP provides case management and social work services for program youth in accordance with all applicable, contractual, and regulatory requirements and accreditation standards.

Duties and Responsibilities

Provide case management services for up to 6 youth living in the ILP program home. Collaborate with community resources to address a variety of youth client needs, including mental health, substance use, housing, employment, and education.

Assume primary responsibility for reviewing referrals and interviewing referred youth for the ILP program per contract guidelines. Provide intake and initial placement services for all newly referred cases.

Provide day-to-day direction and guidance to assigned staff in the ILP home ensuring agency service provision to clients and referral sources required by the contract funding bodies are met.

Take primary responsibility for the upkeep and maintenance of the ILP home, directing and managing assigned staff in order to ensure youth have an inviting and well maintained home.

Work collaboratively with referral sources, agencies, institutions, and individuals who may have a role with each youth in the program.

Coordinate placement arrangements for youth exiting the ILP program to ensure successful transitions in to fully independent living arrangements.

Complete all required reports, treatment plans and correspondence in keeping with high professional writing standards. Audit case records at least quarterly to ensure accuracy with prescribed services and contract standards. Collect, assess, and document appropriate data and client progress.

Attend all scheduled court hearings on assigned caseload. Provide professional testimony regarding client progress and services provided.

Assist in the maintenance of information and data management systems by providing information and statistical reports, as required.

Engage in advocacy and education oriented toward improvement of services to youth clients.

Education, Training, and Licensure/Certification

Bachelor degree in Social Work or related human service field.

Licensed by the State of Michigan preferred

Experience

One year experience in child welfare, preferred.

Additional Work Requirements

Required to participate in a system of 24 hour availability to clients, foster, and adoptive parents, as required.

Unrestricted, valid driver's license with good driving record. Reliable personal transportation suitable for safely transporting persons served; proof of current automobile insurance.
