Job Overview Community Health Nurse (RN):
Join our client as a Community Health Nurse in Queens, NY, and make a meaningful impact on patient care. In this role, you ll provide skilled nursing and comprehensive clinical assessments for PACE program members, primarily in their homes. Collaborating with the interdisciplinary team (IDT), you ll ensure seamless coordination of care in homes, hospitals, nursing facilities, and the PACE center, all while supporting members in achieving their health goals.
Compensation: $105,000 - $118,000/year
Location: Queens, NY
Schedule: Monday to Friday, 8:30 AM - 5:00 PM

Responsibilities as the Community Health Nurse (RN):
- Nursing Care: Provide holistic nursing care, addressing physical, psychological, and social factors.
- Assessments and Planning: Conduct UAS-NY assessments and collaborate with the IDT to create individualized care plans.
- Care Coordination: Ensure continuity of care by working with internal and external healthcare providers.
- Medication Management: Administer prescribed medications and educate patients and caregivers on proper usage.
- Health Education: Teach members and families about treatment plans, medication, and health maintenance.
- Compliance: Maintain accurate documentation and adhere to all regulatory standards.
- Education: Degree from an NLN-accredited school of nursing required (BSN preferred).
- Licensure: Current New York State RN license required.
- Experience: At least 1 year in CHHA, acute care, medical-surgical, or critical care nursing, with required home care or field nursing experience.
- Skills: Strong clinical assessment and care planning abilities, with a focus on patient-centered care.
- Technical Proficiency: Proficient in documentation systems, with the ability to use electronic health records effectively.
- Requirements: Current driver s license required for community visits.
