NVCC Long Term Services Support Case Manager - Company Car

Job Purpose : The purpose of the Nevada Care Connection Long Term Services Support Case Manager position is to provide services assistance and long term case management to seniors, individuals with a disability of any age and caregivers to individuals who have a higher level of need for services navigation with an ongoing evaluations of the services currently being provided as well as identifying additional services and the monitoring of progress toward the outcome of goals established in the consumer's service plan.
The position requires extensive travel to Nevada counties. This position will be out of the office at a minimum of twice a week.
Flexibility with work schedule based on each department's needs . A Company Car will be provided.
Duties/Responsibilities:
Client Support:
Completion of an initial assessment and reassessment at six (6) months identifying key services that are needed.
Performs home visits at a time that best meets the consumer's schedule if applicable and approved by NVCC assistant director.
Establishing a long-term case management plan with each consumer that is referred to as long term case management.
Gather sufficient information to accurately identify and clarify client problems and needs, explore needs beyond the presenting problem, look at short- and long-term solutions, check in with the client and summarize what they are requesting.
Provide one-on-one, telephone, and or email service delivery case management, at AHN offices and other agency locations.
Feel comfortable conducting home visits.
Documentation and Reporting:
Document case notes to each contact with, or on behalf of, the client, including referrals, applications and forms completed, and outcomes. Narrative notes must also include date of entry, brief summary of pertinent information, initials, and title of person making the entry into various databases.
Provide follow-up (in person/phone/email) on a monthly basis to determine outcomes and provide additional assistance in locating resources and arranging services.
Advocacy and Education:
Serve as a visible and trusted source of information on the full range of information of long-term care options, referral, and advocacy.
Inform and educate consumers about their options; assist in linking them to programs and services and provide assistance with various types of applications.
Advocate on behalf of the client/client's family with agencies and service providers.
Partnership Development:
Partner with various partners in private and public sectors.
Participates in program development, marketing and outreach activities including representing NVCC at public information fairs. Outreach and service areas will include the following: Carson, Douglas, Churchill, Elko, Eureka, Humboldt, Lander, Mineral, Pershing, Storey, Washoe, and White Pine Counties.
Schedule and attend outreach visits to offsite locations monthly or on as needed basis.
Gain clear and working knowledge of social services available throughout our communities; establish and maintain appropriate working relationships with partners and agencies involved.
Attend community meetings as requested.
Education Required:
Bachelor's degree from an accredited college or university in social service or public administration, social work, public health, or related field; or combination of education and experience.
Obtain Nevada MAP certification within four (4) months of hire date.
E
xperience Required:
Minimum three (3) years' experience in case management or care coordination.
Strong verbal and communication skills
Minimum three (3) years computer experience including Microsoft operating systems, word processing, spreadsheets, database input and report generation.
Must have knowledge of community resources.
Required to pass a background check through the State of Nevada
Must have a valid driver's license and be able to drive the AHN Company Car when needed.
Excellent customer service skills
Excellent communication skills, both oral and written
Excellent organizational skills
Experience Preferred:
Bilingual preferred but not required.
Physical Requirements:
Be able to stand between 3 - 6 hours per day.
Be able to travel at minimum 2 x a week.
Be able to sit at a computer or sit in the car for up to 4 - 6 hours per day.
Able to drive for up to four (4) hours at a time.
Must be able to lift, carry up to 30 pounds with a fair consistency.
The noise level in the work environment is usually moderate and the employee usually works in a climate-controlled office environment.
