THIS IS A PART-TIME POSITION, EVENINGS AND WEEKENDS REQUIRED.
Qualifications • High School Diploma; • Three years of work experience with consumers with developmental disabilities; • Must be at least 21 years old; • Valid Pennsylvania Drivers License with acceptable driving record as determined by Motor Vehicle Report and insurance guidelines. • Reliable vehicle with valid insurance coverage and inspection as required by state law. • Reliable smart phone as required for electronic verification of visits.
Essential Functions : • Interested applicants must possess a valid driver's license and acceptable driving record, and an insured, reliable vehicle. Three years of driving experience is required. Must be willing to use personal vehicle for work purposes. Mileage reimbursement is provided. • A smart phone is also required for service delivery and documentation. A monthly cell phone stipend is provided. • Completes well written and detailed daily service notes via electronic record system. Must be able to navigate the agency's electronic records. Training on electronic records systems is provided. Requires the use of personal cell phone. • Services are delivered in accordance with the individual's service plan and in relation to their identified outcomes. Staff are responsible for providing services as detailed in the Individual's Service Plan. • Promotes a safe and caring environment where individuals can learn and grow while enjoying a healthy and productive lifestyle. • Promotes community awareness by demonstrating responsible, courteous behavior and a positive perception of individuals with developmental disabilities. • Maintains a professional appearance and demeanor while representing Keystone Community Resources. • Participates in the Interdisciplinary Team Meeting process as appropriate. • Interacts and speaks professionally with individuals, family members, Supports Coordinators, Guardians, Supervisors and other stakeholders. • Implement the daily activity schedule for community outings and events. • Supports an environment that allows for a productive lifestyle for participants by providing community activities. • Ensures the overall health and safety of the clients while out in the community and during transport through safe driving. • Promotes community awareness by demonstrating responsible, courteous behavior and a positive perception of individuals with developmental disabilities. • Accountable for consumer finances while assisting consumers with their personal finances by providing the necessary documentation for income, expenses and /or corporate funds as directed. • Complete and submit incident reports as defined in policy. • Practice safe and sound crisis prevention and intervention strategies only as a last resort and only when there is clear potential for injury by insisting on preventative activities and early intervention. • Provide personal care to clients as the need arises. • Attend staff meetings as scheduled. • Meet regulatory and agency in-service training annually.
