Assesses, plans, organizes, and administers rehabilitative programs that improve mobility, relieve pain, increase strength, and improve or correct disabling conditions resulting from disease or injury.
Qualifications:
- Education: Graduate of an accredited therapy program
- Experience: 2-3 years of related experience; or an equivalent combination of education and experience
- Licenses/Certifications: Current Physical Therapist state license; CPR certification

Requirements
- Restores patient's function, alleviates pain and prevents disabilities by planning and administering medically prescribed physical therapy.
- Provides quality care by assessing and interpreting evaluations and test results.
- Develops a treatment plan for each patient based on initial and ongoing evaluations, including short- and long-term goals, collaboration demands of other staff members, and required patient and caregiver involvement.
- Determines physical therapy treatment plans in consultation with physicians or by prescription.
- Implements Plan of Treatment on a consistent schedule and adjusts this schedule and plan according to patient progress.
- Helps patient accomplish treatment plan by administering manual exercises, massage, and using equipment as necessary.
- Evaluates mobility, strength, and current level of patient function, while setting goals to improve in these areas that are scaled to patient abilities and realistically attainable.
- Evaluate and record patient progress, modify treatment plans, and try new treatments if required.
- Evaluate the use and fit of prosthetic and orthotic devices by observing, noting patient's progress, and recommending adjustments and modifications.
- Documents patient care services according to approved medical documentation and facility documentation procedures.
- Reports patient progress to the patient, patient caregivers and loved ones, facility staff, and healthcare professionals involved in ongoing treatment/care of patients.
- Develops a plan for patients following completion of their Plan of Treatment with appropriate in-home care services.
- Leads by example and exhibits professionalism at all times.
- Holds themselves accountable and encourage collaboration and flexibility within the department and interdepartmental teamwork for positive outcomes.
