Job Summary:
The Advanced Practice Provider will collaborate with physicians and a multidisciplinary team to provide Head and Neck care for patients at the Upstate Cancer Center. Working in collaboration with the attending physicians, the APP will provide comprehensive health assessments and develop plans of care for patients to include medical health histories, physical examination, developing plans based on clinical diagnoses and problems, analyzing diagnostic testing and laboratory results, and ordering interventions based on clinical assessments. Incumbent will work with the team to ensure appropriate follow up is provided and will work collaboratively with the physician group to refill prescriptions, respond to in-basket requests, answer patient questions and complete necessary patient paperwork. Incumbent will actively participate in quality improvements working within teams and committees.
An APP is a member of the multidisciplinary healthcare team who demonstrates professionalism and leadership within the institution. The Nurse Practitioner exemplifies the Upstate mission, vision and values and acts in accordance with Upstate policies and procedures.
Minimum Qualifications:
Master's Degree as a Nurse Practitioner or Physician Assistant or eligible.
NYS licensed/registered NP or PA or eligible required.
Current National Certification consistent to the clinical practice also required.
NPI number and be Medicare enrolled with a Medicare Identification Number (or PTAN) (or eligible for Medicare Enrollment within 6 months of hire).
CPR-BLS Certification required at the time of appointment.
Preferred Qualifications:
Prior experience in specialty area preferred.
