Job Summary:
The Advanced Practice Provider will provide care to patients within the Inclusive Health Services clinic at the Nappi Wellness Center. The APP will provide management of primary and acute care to patient in the outpatient setting. Responsibilities include providing expert level clinical care & coordination of care via a full range of services that will improve quality of patient care assessing & evaluating patients, ordering appropriate laboratory testing & diagnostic studies, arranging for referrals, testing, consultation & therapeutic services, and maintaining outstanding relationships with families, referring physicians & all members of the health care team.
Minimum Qualifications:
NYS licensed/registered NP or PA required. Master’s Degree as a Nurse Practitioner or Physician Assistant & current National Certification consistent to the clinical practice also required. NPI number and be Medicare enrolled with a Medicare Identification Number (or PTAN) (or eligible for Medicare Enrollment within 6 months of hire).
CPR-BLS Certification required at the time of appointment.
Preferred Qualifications:
Two years or greater in primary care.
Work Days:
Monday - Friday, Days with occasional evening rotation.
