Nature of Work:Performing inter-rai assessments in community settings for a variety of programs that serve people with disabilities and seniors. Schedule with clients, their caregivers, family members and/or interested service providers (when appropriate) to meet and complete the applicable evaluation/assessment. Strive for accuracy and timeliness in assessment and referral. Communicate regarding the worker’s recommendations with clients, caregivers and providers. Meet and discuss cases with the multi-disciplinary team. Enter and maintain correct information in the Long Term Care Services and Supports system (LTSS) and Advanced Information Systems Management (AIM). Complete all ancillary documentation and follow-up, as well as submit billing sheets when necessary. Maintain adequate productivity, with a goal of 25 completed considered excellent in a month. Maintain RN license as per Maryland Board of Nursing.Minimum Qualifications
Education:
Associate degree in nursing from an accredited college or university.Experience:Two years in a non-hospital setting.License or Certification:Registered NurseDesired or Preferred Qualifications:Experience in long-term care, home health and community health.
