St. Lukes is also a Magnet hospital for nursing excellence.Established team of 1 physician and 1 Advanced PractitionerDiabetes Self-Management program staffed with Certified Diabetes Educators (RNs and Dietitians)Healthy Living Program, a medically managed weight loss program supported by Dietitians and a Health CoachExperienced staff, both clinical and non-clinical, who know the patients and the communityFull-time position, including dedicated administrative time24/7 hospitalist program is responsible for adult inpatientsFully integrated system with clinics and hospitals on EPIC EMR to improve care coordinationResponsibilities:
Outpatient clinic only; Adult patients 18+Clinic hours Monday Friday 8AM-5PMNo nights or weekendsShared call amongst the providers. Triaged through MyNurseOpportunity to work with patients with diabetes on an outpatient basis.Provide diabetes education and management to our patients and familiesFocus on planning, supervising treatments, and monitoring of diabetes related health concerns.Evaluate and treat multiple endocrine diseases.Work in collaboration with Diabetes Educators from an ADA Accredited Diabetes Self-Management program.Coordinating care of patients and working in collaboration with the patients primary care provider.Exercises independent judgements in the assessment, coordination, management and evaluation of certain medical processes and procedures, under the guidelines of the scope of practice and medical protocols.Ability to work as a team member to improve the health of the people and communities we serveQualifications:
