Summary This is a Direct Hire Open Continuous Announcement (OCA) for Nurse (Clinical Care Coordinator) for the Phoenix Indian Medical Center and will report to the Supervisory Nurse. The Nurse Clinical Care Coordinator ensures patients at the Phoenix Indian Medical Center receive quality patient centered care that is consistent with the patient-centered medical home principles, and that care is timely, appropriate, cost effective, sensitive and coordinated to affect desired clinical outcomes. Responsibilities Implements and initiates standardized disease management and health prevention guidelines across the patient care continuum. Collects subjective and objective health status data from multiple sources, which includes the patient, family members, team members and documented records. Develops and maintains patient's registries based on disease, health condition and/or health prevention initiatives. Interventions delegated to nursing/health care team members are based on their demonstrated competency, state practice acts, available evidence-based guidelines, regulatory and governmental standards and organization policy. Qualifications To qualify for this position, your resume must state sufficient experience and/or education, to perform the duties of the specific position for which you are applying. BASIC REQUIREMENT(S): Education: A graduate or higher level degree, bachelor's degree, associate degree, or diploma from an accredited professional nursing educational program is required. This education must have been accredited by the Commission on Collegiate Nursing Education, Council on Accreditation of Nurse Anesthesia Educational Programs, Accreditation Commission for Midwifery Education, or an accrediting body recognized by the U.S. Department of Education at the time the degree was obtained. Degree From Foreign Nursing School: Official certification from the Commission on Graduates of Foreign Nursing Schools is required for individuals who graduated from foreign nursing schools. Licensure: Applicants must have passed the National Council Licensure Examination In addition, they must possess a current, active, full, and unrestricted license or registration as a professional nurse from a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States. Experience: The work experience must have equipped the applicant with the particular knowledge, skills, and abilities to successfully perform the duties of the position. At the GS-9 and above, many positions may require experience in a specialty area of nursing. GS-10 Basic Requirements: Possession of a diploma, associate degree or bachelor's degree in Nursing from a professional nursing program and at least one full year of professional nursing experience (equivalent to the GS-9 grade level): Implements and initiates standardized disease management and health prevention guidelines across the patient care continuum. Responsible for setting up and monitoring patients enrolled in disease specific home tele-health monitoring and communicating clinical information and changes to clinical plan between patient and provider. Develops a plan of care with the care team according to clinical practice guidelines with the patient, family, caregiver(s) and significant other(s). Identifies resources to integrate additional clinical practice guidelines to support continuity of care. Education This position has an education requirement. Additional Information Federal Employees earn annual leave at a rate (4, 6 or 8 hours per pay period) which is based on the number of years they have served as a Federal employee. IHS may offer newly-appointed Federal employees credit for their directly related previous non-federal experience or active duty uniformed military service. This credited service can be used in determining the rate at which they earn annual leave. This position is covered by a Bargaining Unit.
