We are looking for a licensed Speech Language Pathologist to join our skilled nursing facility team. As the Speech Language Pathologist, your role involves assessing and treating residents with communication and swallowing disorders to enhance their quality of life and functional abilities.

Responsibilities:

Evaluate residents' communication and swallowing abilities using standardized assessments.

Create and execute personalized treatment plans based on assessments, resident objectives, and physician directives.

Deliver skilled speech therapy interventions, covering articulation, language therapy, cognitive-communication therapy, and swallowing therapy.

Monitor and record resident progress, adjusting treatment plans when needed.

Collaborate with interdisciplinary team members, such as physicians, nurses, occupational therapists, and physical therapists, to coordinate care and ensure optimal resident outcomes.

Educate residents and families on speech therapy goals, treatment plans, and home exercises.

Maintain accurate and timely documentation of speech therapy services.

Participate in team meetings, educational sessions, and quality improvement activities.

Stay updated on speech therapy practices and relevant regulations.

Qualifications:

Current Speech Language Pathologist license in the state of employment.

Minimum of 2 years of healthcare experience, preferably in a skilled nursing facility.

Knowledge of current speech therapy standards and regulations.

Strong communication and interpersonal skills.

Ability to collaborate effectively within a team.

Skill in managing multiple priorities and meeting deadlines.

Proficiency in electronic medical records and other healthcare software.
