Experienced staff, both clinical and non-clinical who know thepatients and the community

Full-time position including dedicated administrative time, withflexible scheduling to ensure sufficient patient accessFully integrated system with clinics and hospitals on EPIC EMR toimprove care coordinationResponsibilities:

Average of 15-18 patients per day; no nights or weekendsShared call amongst the providers a week at a time (Sat. & Sun. Call). Triaged through MyNurseAbility to work as a team member to improve the health of the people and communities we serveEvaluates, interprets and coordinates information to make decisions and recommendations regarding patient carePatient-focused, friendly and willing to collaborate in a team environmentExcellent communication (written and verbal), analytical and problem-solving skillsQualifications:

Nurse Practitioner or Physician AssistantBoard Certified3-4 years of experience preferredHold an unrestricted medical license in the state of IllinoisHold current and unrestricted state and federal authority to prescribe and administer medications as necessary or appropriate to the Advanced Practitioners field of practiceEligible to work in the United StatesWe OfferInitial 2-year salary guarantee with ability to exceed on an innovativecompensation program
