Medical Coordinator

Community Options, Inc. is a national non-profit agency providing services to individuals with disabilities in 12 states.We are seeking a Medical Coordinator in Nashville, TN ! The Medical Coordinator will plan, track, monitor and ensure that all clients' needs are being met within appropriate time frame. Primary responsibilities include the scheduling of all annual, follow-up, and emergency medical appointments. These include communicating with medical professional staff, program specialists, supervisors and program directors. ResponsibilitiesSchedule all client medical appointments to state mandated licensing regulationsObserve and monitor the client's status through paperwork and follow-up appointmentsObserve and track clients' physical care including, but not limited to annual and emergency appointmentsProvide accurate, timely, and consistent documentation of client medical appointments in the appropriate recordsAttend all pertinent medical appointments that need to be addressed more clearly and precise to achieve the best results for the individualCommunicate with supervisors, co-workers and medical professionals' regarding clients' statusEmployees must cooperate with the licensee and department staff in any inspection, inquiry or investigationMinimum RequirementsHigh School Diploma or GED; bachelor's degree preferredValid driver's license with a satisfactory driving recordComplete all state and agency required training per state guidelinesAbility to establish a comfortable and supportive relationship with individuals receiving supportsAbility to exercise good judgment and remain calm in crisis situationsWorking ConditionsMay be required to be on-call in cooperation with other management staffFrequent lifting, stretching, and other physical exertion may be requiredMay be required to transport individuals utilizing your own vehicle or company provided vehicles May be required to lift or move 25+ poundsMay assist with wheelchair transfer of non-ambulatory individualsMay be exposed to various medical conditions and communicable diseaseWhy Community Options?Competitive Insurance Benefits (Medical, Dental, Vision)Paid Holidays Including a Birthday HolidayGenerous PTOEmployee Incentive & Discount Programs403b Retirement PlanIncredible career growth opportunitiesSend resume to: Resumes-TN@comop.orgCommunity Options is an Equal Opportunity Employer M/F/D/V
