Participate in all phases of care coordination.Provide nursing expertise about the coordination of care process, including assessment, planning,implementation, coordination, and monitoring. Identify opportunities for care coordination andidentify and integrate local processes.Develop and implement local strategies using inpatient, outpatient, onsite and telephonic followups.Develop and implement tools to support case management, such as those used for patientidentification and patient assessment, clinical practice guidelines, algorithms, Care Point and MHSGenesis software, and databases for community resources.Integrate coordination of care and utilization management and integrating nursing casemanagement with social work case management.Qualifications:

Possess an Associate degree from an accredited nursing programThe qualified Registered Nurse will, have a minimum of 1 year nursing experience.Valid Registered Nursing license from any US jurisdictionBasic Life Support certification (BLS) from an American Heart Association Program or the American Red Cross
