A healthcare network in New York City is currently seeking a licensed Nurse Practitioner (NP) to join their growing team in Manhattan. About the Opportunity: Schedule: Monday to Friday Hours: Standard business Specialty: ISNP / Managed Care Setting: Case Management Department Patient Population: Geriatric Responsibilities: The Nurse Practitioner (NP) will: Provide and/or coordinate primary clinical care services for assigned high risk members and facilitate supportive care during transitions from acute or sub-acute care setting. Collaborate with Plan Case Manager to schedule and follow-up home visits to update the care plan Ensure safe transition from setting to setting, conducts post discharge assessment within a reasonable time periodEvaluate appropriateness of member transfers for acute inpatient services, prospectively and retrospectively and share necessary clinical information with collaborating physicians/facilities Provide individualized care for the at risk population that would improve care coordination, reduce avoidable utilization of hospitals, improve the health and quality of life of community dwelling members Perform other duties, as needed Qualifications: Bachelor's and/or Master's Degree NYS Nurse Practitioner (NP) license Desired Skills: Previous experience in an Acute, Subacute, and/or Long Term Care setting Managed Care background ISNP or Medicare Advantage experience
