In-Person Clinic, some fieldJOB PURPOSE:
Under the direction of the Chief Medical Officer, the Site Physician is responsible for the professional, organizational, and administrative aspects of providing quality medical care to CLHC/Day Treatment Center patients.
QUALIFICATIONS:
Education:Graduate of an accredited school of medicine.Board Certification in Internal Medicine or Family Practice.Board Certification or experience in Geriatric Medicine.Experience:
Residency in Internal Medicine or Family Practice is required.Fellowship, and diploma or experience in geriatrics and palliative care is preferred.Board certification in Internal Medicine or Family Practice is required.No malpractice or loss of license at the DTC.Excellent verbal and written communication skills.Minimum of one (1) year of experience working with a frail or elderly population or, if the individual has less than one (1) year of experience but meets all other requirements, must receive appropriate training from the PACE organization on working with a frail or elderly population upon hiring.License:
Current active unrestricted license to practice medicine in NYS.
Additional Requirements:
Be legally authorized (for example, currently licensed, registered, or certified if applicable) to practice in the State in which the healthcare professional will perform the function.Be medically cleared for communicable diseases and have all immunizations up-to-date before engaging in direct participant contact.Must have a Medicaid Provider ID and a Medicare Provider ID in good standing.
