Advanced Practice Provider for Cardiology Heart Failure Program - Denver, CO
Your day.
SCL Health Medical Group is seeking an experienced Nurse Practitioner or Physician Assistant for our Heart Failure Clinic. The selected candidate will partner with the two board certified Heart Failure Cardiologists to continue to build a pre-eminent Heart Failure Program in the state of Colorado.You would practice at our Cardiology clinic and Saint Joseph Hospital in Denver, ColoradoHours are Mon-Fri, no call and no weekendsYou would work with stage 3 and 4 heart failure patients to ensure medication compliance, monitoring dietary and lifestyle changes, regular follow ups and much moreThis position would be part of the continuity of patient care from hospital to clinic and back to the primary care physician or cardiology service lineYou would work a combination of outpatient clinic consults and follow ups along with inpatient rounding and consults

But we also expect excellence, which is why we require:Graduate from an accredited Physician assistant or Nurse Practitioner programLicensed as an NP or PA in the state of CO or able to obtainDEA and full prescriptive authority in CO or able to obtainBLS and ACLS requiredMinimum of 2 years of experience as an NP or PA in a cardiology setting (PLEASE NO new grads)Experience in a Heart Failure program, highly desired
