A health services organization in Connecticut is currently seeking a licensed Nurse Practitioner (NP) and/or Physician Assistant (PA) for a great opportunity with their Primary Care team in New Haven County.
Responsibilities:
The Primary Care NP / PA will be responsible for:Examine, obtain medical history and treat acute and chronic illnessesOrder, perform and interpret diagnostic testsCreate treatment plans, and involve patients and family members in those plansInitiate appropriate specialty referralsCollaborate on improvement in care delivery and service with management, other Physicians and staffWork with the Care team to close care gaps and improve quality measuresWork with practice management to ensure compliance with billing and coding requirementsPerform other duties, as neededQualifications:
1+ year of Primary Care experienceGraduation from accredited APRN or PA programCT Nurse Practitioner (NP) and/or Physician Assistant (PA) licenseDEA registration and Controlled Substance certificationSolid assessment, clinical, and documentation skillsGreat interpersonal skillsExcellent communication skills (written and verbal)Strong attention to detailHighly organized
