Qualifications:

Associate's Degree in Human Services or equivalent credits preferredMinimum two years' experience in ID/DD or related human services backgroundSupervisory experience preferredKnowledge of OPWDD HCBS Waiver ServicesExcellent verbal and written communication skillsValid Driver's License requiredAbility to work a flexible work scheduleComputer Literacy requiredSome Responsibilities

Assist in the day to day oversight of program operations as it pertains to individuals, staff and program siteMake decisions in conjunction with and in the absence of the Day Habilitation Supervisor that drive operational processesProvide ongoing training and coaching to all Direct Support Professional staff and assist with personnel related issuesAssist with the implementation of emergency protocolsEnsure adequate staffing at all timesParticipate in recruitment of program staffProvide ongoing protective oversight to individualsAssist in development, approval, implementation, scheduling and participation of the Day Habilitation Plans and meetingsProvide assistance and instruction in basic activities of Daily Habilitation ServicesDrive agency vans and demonstrate safe driving practicesEnsure proper maintenance, repair and cleanliness of all program vehiclesMonitor Fleetmatics GPS reports, ensure follow-up with all Motorist Observation Reports and provide data for safe driver incentive.Ensure a clean and safe physical plant environmentEnsure for the completion of quarterly internal physical plant checksConduct monthly fire evacuation drills and record data in safety logAttend all required in-service training (i.e., SCIP, CPR) and staff meetingsDevelop and maintain positive working relationships at volunteer sites and with family membersProvide daily supports to staff and ensure operational and regulatory compliance is maintained at all timesEnsure program activities are Person Centered and based on the individual's valued outcomesAdhere to all regulations and agency policy and procedures with regard to incident reportingConduct field based quality spot checks (at least monthly)Ensure review of daily activity schedules and electronic data collection (Medisked)Reconcile Wells Fargo and gas receiptsCollaborate with Department Executive Assistant to ensure pertinent documents (i.e., ISPs, LOCs) are entered into electronic systemPerform other duties as delegated, including assignments at multiple program locationsAbout Life's WORC:
