Mount Sinai South Nassau provides a salary range in good faith determination of potential compensation to comply with the New York State law on Salary Transparency in Job Advertisements. Position Summary

Summary:The Population Health Program Coordinator I is responsible for providing administrative, operational, and programmatic support services to an assigned program area. The Program provides a specific service or carries out specific activities as a separate entity or as a component program within a department but is not a department itself. The Program is ongoing but may have a business cycle. The program coordinator also oversees an assigned group of patients and acts as the primary liaison between the patient and the program to which they are assigned. This individual ensures that the patient is connected to prevention and treatment services and ensures that the patient adheres to the established plan of care.

Qualifications

Responsibilities:Education Required• Must complete in-house orientation program• Bachelor's Degree or• Equivalent combination of education and/or experience may substitute for the degree when the experiences are closely related to the duties of the job

Experience And Skills• Excellent interpersonal skills• Speaks, reads and writes English• 1-2 years management experience preferred• Basic Microsoft Office skills• Excellent written and oral communication skills• Excellent organizational skills and the ability to prioritize multiple tasks, projects and assignments• Exceptional attention to detail and accuracy• Bilingual in Spanish a plus
