SUMMARYThis position is as an active member of a community-based integrated service delivery network providing a full spectrum of physical health and behavioral health services. The Primary Care Family Nurse Practitioner (FNP) will assume clinical decision-making responsibilities in the assessment and management of the physical health disorders of clients referred. These services are provided under the supervision of the Chief Medical Officer.
DUTIES
Assumes responsibility for the physical health evaluation and assessment of assigned clients.
Assumes responsibility for the pharmacologic management of clients referred as well as for collaboration with Behavioral Health Medical Providers (BHMPs) regarding overall medication management.
Consults with ancillary services, physician specialists, and outside agencies in order to assist clients and families in attaining maximum good health.
Works closely with integrated care teams to ensure effective coordination of care and positive health outcomes.
Shares responsibility with administrative, medical, nursing and behavioral health personnel for the quality and efficiency of client care services.
Documents all work in the electronic health record on the day of service provision.
Participates in peer review with other professional staff members.
Complies with agency policies and procedures.
Maintains appropriate documentation
Review and justification of treatment interventions.
Formulation of diagnostic work-up or therapeutic plan.
Significant changes in the client's condition.
Major therapeutic interventions.
Modification of diagnostic work-up or therapeutic plan.
Consistency with Agency policies and procedures regarding documentation.
Participates as an interdisciplinary team member
Assist team members in recognizing important changes in the client's condition.
Assist team members in implementing the interdisciplinary plan of care of the client.
Enhance the team's ability to establish a dialogue with the client about care and treatment.
Employee must be able to monitor consumer activities, with or without a reasonable accommodation.

REQUIREMENTS
FNP degree completed at an accredited training program.
Board eligible or certified, and licensed with the Arizona Board of Nursing and Drug Enforcement Administration.
Practitioner in good standing with the agency with practice privileges consistent with clinical qualifications.
Experience working in managed care settings.
Experience working with clients who have serious and persistent mental disorders.
Experience working with clients who have co-occurring disorders.
Experience in the treatment of traumatic brain injured clients.
Experience in the psychiatric treatment of the developmentally disabled.
Experience working in case management driven programs.
Experience in community based behavioral health programs.
Additional qualifications in substance abuse treatment.
KNOWLEDGE,SKILLS, AND ABILITIES
Able to maintain a calm, supportive attitude during crisis or potential crisis situations.
Able to interact with behaviorally challenged clients and families, professional and support staff, and to assist in making community referrals.
Demonstrates ability to initiate and participate in employee, client, and family education.
Able to work within a multidisciplinary structure.
Knowledge of resources available in the community.
Excellent communication skills, and fluent in English both verbally and in writing.
Able to effectively employ analytical and problem-solving skills.
Able to work effectively in a culturally diverse environment requiring understanding of diverse ethnic and racial backgrounds.

Possession of first aid and CPR certification.
