Diabetic patients are seen with 4 co-located Diabetes Educators.• The catchment area of the practice is large and they see patients with all usual and some unusual endocrine problems. Standard endocrinology testing is available, including Insulin Tolerance Testing.• Diabetes care focuses on population health. This part of the program is led by one of the NPs. Some of the components of the program are:o Inpatient responsibilities for the endocrinologists are limited, but will likely increase as their health system grows. The Division of Endocrinology collaborates closely with this program.o They also believe that a responsible healthcare system must, in the face of the epidemic that we are facing, address prevention of diabetes. Endo patients assigned on a 2:1 basis favoring the new Endocrinologist. Decisions are made collaboratively with the broad mission and philosophy of practice. Currently the Division of Endo provides almost no impatient care. All potential inpatient consults are screened by phone (less than 1 per week) and the vast majority of issues are addressed by providing advice on the phone with outpatient follow up. There is no direct inpatient care responsibility. The Division also does not provide direct phone access to patients. All patients have primary care providers and are encouraged to call PCP first. Growth and leadership potential exist across the spectrum of activities. Leadership help / assumption of leadership in any of the areas of activity would be welcome.
