JOB SUMMARY Responsible for evaluation, planning, directing and administering physical therapy modalities of treatments, in the home, as prescribed by a licensed practitioner. Administers treatments and physical agents as prescribed by a licensed practitioner in effort to restore function and prevent disability following injury, disease or physical disability. Assists patients to reach their maximum performance and level of functioning, while learning to live within the limits of their capabilities. The staff physical therapist coordinates, delegates, and supervises responsibilities assigned to supportive staff. The staff physical therapist participates in operational aspects of the department while maintaining performance improvement activities within the department and participates in Performance Improvement activities. Provides input in formulating budget, assists in evaluating department performance versus budget controls and takes appropriate action to remain within budget guidelines. Ensures that changes of the treatment plan are accurate and entered on a timely basis. Participates by all infection control, departmental equipment training, and organizational safety issues.
Job Specific: 1. Responsible for the clinical direction of Physical Therapist Assistants.(EF) 2. Ability to observe and evaluate treatment effect and recommends change as needed. (EF) 3. Ability to perform an appropriate evaluation on all patients as related to the referral and diagnosis. This includes neonate, pediatric, adolescent, adult and geriatric patient population. (EF) 4. Formulates a teaching plan based upon identified learning needs, home needs, and evaluates/records effectiveness of learning, family is included in teaching as appropriate. (EF) 5. Demonstrates knowledge of physical therapy modalities. (EF) 6. Treats patients and their families with respect and dignity. Identifies and addresses psychosocial, cultural, ethnic, and religious/spiritual needs of patients. (EF) 7. Manages and operates equipment safely and correctly. Maintains clean and safe working environment in the home. (EF) 8. Interacts professionally with patient/family and involves patient/family in the information of the plan of care. (EF) 9. Communicates appropriately and clearly, to licensed practitioner, staff and administration team. (EF) 10. Coordinates and directs patient care to ensure patients' needs are met and organizational policy is followed. (EF) 11. Maintains records pertinent to personnel and operation of the Home Health department (EF) 12. Maintains a good working relationship both within the Home Health department and within other departments. (EF) 13. Consults other professionals and departments as appropriate to collaborate in patient care and performance improvement activities. (EF) 14. Documentation meets current standards and policies. (EF) 15. Assists with orientation and in-service training for certified/licensed Home Health staff members, participates in evaluation/guidance and educational programs. (EF) 16. Comply with home health State and Federal Regulations as applicable (EF)
Hospital: 1. Adheres to organization-wide and other applicable policies and procedures. 2. Day-to-day performance complies with the hospital's Service Excellence Behavioral Expectations. 3. Performs within the prescribed limits of the hospital's Ethics, Compliance and Confidentiality Program guidelines. 4. Performs within his/her professional Scope of Practice at all times.

Minimum Requirements
EDUCATIONAL REQUIREMENTS Required: Graduate of a Physical Therapy Program approved by the Texas State Board of Physical Therapy Examiners. EXPERIENCE Required: None
Preferred: Prefer over one year of combined exposure to acute orthopedic, neurology, skilled nursing patients, out-patients, home health and wound care. LICENSURE / CERTIFICATION REQUIREMENTS Required: Licensed by the Texas State Board of Physical Therapy Examiners and BCLS certification required.
