JOB SUMMARY Responsible for administration and direct application of Speech/Language Pathology treatment modalities, in the home, as prescribed by a privileged referring licensed practitioner. Evaluates, plans, directs and treats all patients appropriately referred to speech/language pathology for treatment to restore function and prevent disability following disease or injury. Assists patient to obtain maximum performance and function in society while learning to live a routine "normal" life within the constraints of their disability. Participates in performance improvement activities within the department and participates in QM activities. Assure personal competency. Provides information related to departmental budget considerations in relationship to Speech Pathology needs for optimum patient care and service. Assists in scheduling of patient visits for speech pathology evaluation and treatment. Participates in both departmental and organizational in-services.
JOB DUTIES AND RESPONSIBILITIES Job Specific: 1. Ability to observe and evaluate treatment effect and recommends change as needed within a home environment. (EF) 2. Ability to perform an appropriate evaluation on all patients as related to the referral and diagnosis. This includes neonate, pediatric, adolescent, adult and geriatric patient population. (EF) 3. Formulates a teaching plan based upon identified learning needs, home needs, and evaluates/records effectiveness of learning; family is included in teaching as appropriate. (EF) 4. Demonstrates knowledge of speech therapy modalities. (EF) 5. Treats patients and their families with respect and dignity. Identifies and addresses psychosocial, cultural, ethnic, and religious/spiritual needs of patients. (EF) 6. Manages and operates equipment safely and correctly. Maintains cleanliness and safety. (EF) 7. Interacts professionally with patient/family and involves patient/family in the information of the plan of care. (EF) 8. Communicates appropriately and clearly to licensed practitioner, staff and administration team. (EF) 9. Coordinates and directs patient care to ensure patients' needs are met and organizational policy is followed. (EF) 10. Maintains records pertinent to personnel and operation of the agency. (EF) 11. Demonstrates an ability to be flexible, organized and function under stressful situations. (EF) 12. Maintains a good working relationship both within the rehab department and other departments. (EF) 13. Consults other departments as appropriate to collaborate in patient care and performance improvement activities. (EF) 14. Documentation meets current standards and policies. (EF) 15. Assists with orientation and in-service training for certified department staff members, participates in evaluation/guidance and educational programs. (EF) 16. Complies with home health State and Federal Regulations as applicable (EF)
Hospital: 1. Adheres to organization-wide and other applicable policies and procedures. 2. Day-to-day performance complies with the hospital's Service Excellence Behavioral Expectations. 3. Performs within the prescribed limits of the hospital's Ethics, Compliance and Confidentiality Program guidelines. 4. Performs within his/her professional Scope of Practice at all times.

Minimum Requirements
EDUCATIONAL REQUIREMENTS Required: Master's degree in Speech/Language Pathology
EXPERIENCE
Preferred: Prefer one to two years of combined exposure to neuro., skilled nursing patients, out-patients, home health.
Special Skills and Abilities: Required: Maintain professional skills as required by Citizens Medical Center and Texas Department of Health and ASHA
LICENSURE / CERTIFICATION REQUIREMENTS Required: Licensed as/or working toward license as a SLP or intern in the State of Texas. Certified (CCC-SLP) or working toward certification (CFY-SLP) by ASHA. BCLS certification required.
