Faith Presbyterian HospiceRN Case ManagerMonday - Friday8:00am - 5:00pmResponsibilities:ESSENTIAL DUTIES:Models a service attitude towards others, takes ownership in solving problems and takes the initiative to make things better, is friendly and courteous to residents, patients and co-workersEnsure resident/patient, visitor, and co-worker safety by remaining aware of and reporting any new or existing safety hazardsMaintain good attendance recordActively participate in coordination of all aspects of patients care, in accordance with current professional standards and practiceContribute to patient and family counseling and educationDevelop and evaluate the plan of careAssume primary responsibility for a patient/family caseload that includes the admission and assessing, planning, implementing and evaluating phases of the nursing process.When designated, fulfills administrative and supervisory responsibilities and qualifications as required by State and Federal regulations, including, but not limited to, TAC Rules.Initiate communication with attending physicians, other hospice staff members and other agencies as needed to coordinate optimal care and use of resources for the patient/family.Practices Universal Precautions according to OSHA regulations and organizational policyMaintains confidentiality of all patient information, sharing only what needs to be shared with the interdisciplinary team in order for the patient to be adequately care forRespects patient/family rights by: (knocking on doors before entering the residence, addresses patient/family by name, allows patient to make own decisions when appropriate)Provides emotional support and compassionate care to the patient and family. Demonstrates listening skills, offers encouragement when appropriate, adjusts care to the patients physical condition each visitObtain data on physical, psychological, social and spiritual factors that may influence patient/family health status and incorporate that data into the plan of care.Seek input from other team members regarding the patient plan of care to obtain additional knowledge and support.Maintain up-to-date legible patient records so that problems, plans, actions and goals are accurately and clearly stated and changes are reflected as they occur.Instruct/Educate primary caregivers, volunteers, and other staff to provide care as indicated.Attend interdisciplinary team meeting and facilitate discussion of issues from caseload for full staff discussion, consultation and evaluation; Participate in ongoing interdisciplinary comprehensive assessmentSupervise and document observance of the hospice aide delivery of care every two weeks.Inform the Patient Care Manager of unusual or potentially problematic patient/family issues.Assist other nurse/case managers including assuming primary responsibility for patients when necessaryShare in providing 24-hour, seven day week coverage to patients/families when necessary. Is available for holiday, on-call, illness, and vacation coverage as neededAttends and participates in orientation; Participate in QAPI program and hospice sponsored in-service trainingProvide appropriate support at time of death. Reports death to appropriate physician.Follows policies regarding ordering and disposal of patient medications.Plans the administration of and/or administers medication with knowledge of actions, contraindications, side effects and desired outcomes.Participates in agency and community programs as requestedPerforms chart reviews as assignedFollows prescribed dress code, wears name tag appropriately and maintains a professional image to patients and familiesPerform other similar duties as assigned by Patient Care ManagerQualifications:MINIMUM QUALIFICATIONS:Bachelors degree (B.S/B.S.N.) preferred; One to two years related experience and/or training; or equivalent combination of education and experiencePossession of a valid Texas Registered Nurse LicenseProficient ability to communicate orally and in writing in the English languageMinimum of two years of varied work experience as a professional nurse and preferably one of the two years in an acute settingValid Texas Drivers License and reliable method of transportationAbility to work independently; Ability to adjust to changing conditions and frequent demands. Ability to acquire new knowledge to perform additional or more difficult tasksAbility to maintain a positive, stable disposition in performing, working in association with staff, patients and familiesAbility to maintain a high level of performance in an environment of frequent demands and changesAbility to meet new and/or stressful situations with the appropriate responses, ability to work effectively as part of a healthcare team, to share information freely, to use and seek supervision and consultation verbally and in writing, and to be able to follow both verbal and written instructionsAbility to perform Essential Duties as outlined belowGenuine caring for and interest in the care of mature adults, handicapped people and hospice PatientsAbility to perform work tasks within the Physical Demand Requirements as outlined belowAbility to comply with the Dying Persons Bill of Rights and Employee ResponsibilitiesAbility to comply with organizational and departmental safety policies and procedures
