Boone Pickens Hospice and Palliative Care Center.Responsibilities:ESSENTIAL DUTIES:Models a service and solutions oriented attitude towards others, takes ownership in solving problems and takes the initiative to make things better, is friendly and courteous to residents, patients and co-workersEnsure resident/patient, visitor, and co-worker safety by remaining aware of and reporting any new or existing safety hazardsKnowledge of Medicare pre-admissions for qualifying diagnosis and coverage eligibilityObtains/ maintains physician certificates for each Medicare residentMaintain good attendance recordMaintains MDS scheduleCompletes MDS 0 for Medicare residentsAssures that all MDS information is completed and transmitted per policy, and is compliant with all regulations and forwarded to billing officeTracks Medicare resident benefits days, validates daily Medicare census and coordinates this information with accounting representative and therapy representative, to ensure accurate billing (if applicable)Assures all quarterly resident assessments are completedAssures all care plans completed and updated as neededCoordinates daily PPS meetings and weekly Medicare meetingsReviews nursing documentation of residents and provides guidance/training as needed, to ensure charting is appropriate and meets coverage guidelinesResponsible for completing case management to ensure adequate documentation for reimbursementCoordinates discharge of residentsWorks with Clinical Liaison to verify appropriateness of admission and admission diagnosis codesMaintains weekly communication with insurance case manager to receive approval for resident treatment and communicate with physician on insurance resident admission for timeliness of physician visitsCommunicates with rehab department on routine basis, to discuss therapy servicesCommunicates with Life Enrichment and Social Services to coordinate assessment in a timely mannerOther duties as assigned by Director of Nursing

Qualifications:MINIMUM QUALIFICATIONS:Valid Texas LVN/RN licenseAANAC RAC-CT CertificationIn depth knowledge of MDSMinimum of 2 years experience with RAI process/MDS/PPS/Medicaid CMI in a Medicare/Long Term Care settingExperience with electronic medical chartingDemonstrated MDS application skillsKnowledge of SNF, Medicare documentation and reimbursement guidelinesDemonstrated MDS application skillsDemonstrated computer competenciesKnowledge of rehabilitation theory and applicationKnowledge of current professional nursing theory and practiceAbility to read, write and effectively communicate in EnglishExcellent verbal and written communication skillsKnowledge of restorative nursing care practicesDemonstrated supervisory and managerial skillsAbility to maintain a stable disposition in performing work in association with staff and residentsAbility to maintain a high level of performance in an environment of frequent demands and changesAbility to perform Essential Duties as outlined belowGenuine caring for and interest in the care of mature adults, persons with disabilities and hospice patientsAbility to perform work tasks within the Physical Demand Requirements as outlined belowAbility to comply with the Resident Bill of Rights (LTC)
