A home healthcare organization in New York City is looking to add a new, licensed Registered Nurse (RN) to join their growing Medical Respite Program in Manhattan. In this role, the Registered Nurse (RN) will be responsible for conducting psychiatric assessments, assessing physical health needs, and making appropriate referrals to Community Physicians.

The Registered Nurse (RN) will:

Provide comprehensive primary health care, including case finding, health screening, health promotion, physical and psychosocial assessment, case management, and overall management of medicationsDevelop, maintain, and revise written plans of care and general healthcare recordsEngage new clients in the continuum of care through outreachCollaborate with other health care providers and make appropriate referralsServe as client advocate specifically in accessing the healthcare systemProvide communication to team members and participate in community and apartment visits to clientsCoordinate services with other community mental health services such as, inpatient, outpatient, emergency, or day treatment. Assist in the development of a program that closes gaps in the continuum of servicesAssist in the training of team members in the range of health promotion and illness prevention, treatment, rehabilitation, and support services availableAct as a healthcare resource to clients and extended familiesPerform other duties, as neededQualifications:

Graduate of an accredited School for NursingNYS Registered Nurse (RN) licensePrevious experience in Primary and Behavioral Health Intake AssessmentsComputer savvyMicrosoft Office Suite proficient (Excel, Word, etc.)Proficiency with Health-related computer applicationsSolid assessment, clinical, and documentation skillsPatient orientedGreat interpersonal skillsExcellent communication skills (written and verbal)Strong attention to detailHighly organizedDesired Skills:

5+ years of experience working with Adults dealing with Senior Mental IllnessBilingual
