The Physical Therapist (PT) evaluates the physical therapy needs of assigned patients, establishes goals with the patient and plans, develops and implements individual physical therapy programs designed to improve patient mobility and independent function. The PT provides age and developmentally appropriate care to pediatric to geriatric patients. Effectively interacts with patients, doctors and other healthcare members.
Responsiblities:Assess, plan, treat, evaluate and implement a POC for all clients seenDemonstrates understanding of admission process, treatment costs, coding, financial support & appropriate billingCommunicates and interacts with patients, families, physicians and staff demonstrating behavior consistent with the hospital's core valuesCompletes documentation of all patient care activities in a timely manner in the electronic medical record.Ability to transfer and position patients.

1 year Home Health PT experience - REQUIREDValid drivers license and car insurance - REQUIREDCOVID vaccination required - DECLINATIONS WILL NOT BE APPROVED **Please add vaccination status in the profile**OASIS and start of care experience - strongly preferredManager will review local candidates!
