Education and Experience Requirements
Essential Duties and Responsibilities:- Identify training needs and development opportunities for subordinates.- Discuss job performance concerns with subordinates to identify causes and issues and works closely with the HR department on resolving problems.- Participate in the work of subordinates to facilitate productivity or to overcome difficult aspects of work.- Evaluate subordinates' job performance and recommend appropriate personnel action.- Perform in-home and telehealth evaluations of patients that includes evaluation of health status, strengths, care needs, and preferences utilizing the UAS-NY assessment tool.- Facilitate and assist new CFEEC Nurse Evaluators to meet the essential functions of the Nurse Evaluator role.- Conduct CFEEC appointment visits to help train new Nurse Evaluators to provide demonstration and education. Instruct, demonstrate, observe assessment skills and techniques and provide feedback to the Regional Nurse Manager. Raise issues of concern to the Regional Nurse Manager.- Assist with training of new Nurse Evaluators and refresher trainings as assigned for existing nurses.- Identify task weakness in staff and take appropriate action to correct deficiency.- Assist the Regional Nurse Manager in providing assessment guidance to team members.Minimum Requirements:- Current RN Licensure in New York State- 2 yearsˇ RN experience in home care including 1 year RN experience using the UAS-NY assessment tool- Ability to review health status questions and make evaluations and determinations based on UAS criteria for enrollment; ability to work with technology in the workplace.- Knowledge of the theory, principles, and practices of general professional nursing.- Knowledge of state and agency laws and regulations governing general professional nursing practices.
