Compensation
Salary Range: $61.50 - $61.50 / hourThe pay range above represents the lowest possible rate for the position and the highest possible rate. Factors that may be used to determine where newly hired employees will be placed in the pay range include the employee specific skills and qualifications, relevant years of experience and comparison to other employees already in this role. Responsibilities
Position Overview:The RN Case Manager (CM) works with patients and families along with the healthcare team in the collaborative development of a plan for discharge and arrangement any needed post-acute care services. The position assists with gathering data to ensure that patient care provided meets approved admission criteria, continued stay, and appropriate level of care .
Qualifications
Required:Graduate of an accredited school of registered nursing
Current CA Registered Nurse (RN) LicenseSix (6) months of recent full-time acute care nursing experience in an acute care hospital (RN CM I)One (1) year of case management experience (RN CM II)Preferred:Bachelors of Science in Nursing (BSN)Bilingual in SpanishOne (1) year of experience in Case Management and strong discharge planning skillsOne (1) year of experience with InterQualOne (1) year of experience with Allscripts and Meditech
