La Rabida Children’s Hospital is hiring a full-time Pharmacy ClinicalCoordinator for the Inpatient Pharmacydepartment in Chicago, IL.

Job Summary:
The In-patient Pharmacy Clinical Coordinator applies a collaborative approach in working with the interdisciplinary team to serve as the expert in safe medication use. Provides specialized pharmaceutical services including advising physicians on matters about medications and participates in the development and delivery of continuing education programs for Pharmacy, Patient Care Services, Medical staff, and students.
Essential Pharmacy Clinical Coordinator Functions:
Dispensing
Reviews and interprets drug orders and completes or supervises the process of providing medications to hospital patients, clinic outpatients, and hospital employees
Advises or consults with and provides drug information to physicians, nurses, hospital personnel, patients, and patient's families
Provides or supervises the production of IV admixtures, chemotherapy admixtures, or other compounded medications using aseptic technique and following established department policies and procedures
Monitors patients' drug therapies for appropriateness, adverse effects, and possible interactions
Performs quality assurance monitoring, drug use evaluation, drug therapy monitoring, and floor stock duties as assigned
Provides guidance and supervision to Pharmacy Technicians in the joint effort of providing medications and/or supplies to patients and ancillary departments
Non-Distributive
Provide specialized pharmaceutical services to improve drug usage and therapeutic outcomes including but not limited to:
Recommending new or a change in drug therapy
Change in dose regimens
Advising physicians on issues concerning drug therapy
The inherent toxicity of drugs and their side effects
Assisting in the prescription of appropriate doses
Participate in Patient Care Rounds with the multidisciplinary team to evaluate patient progress.
Conduct and evaluate medication histories.
Assist in the admission and discharge medication reconciliation process.
Guide the healthcare team in the proper use of prescribed drugs.
Participates in the organization and maintenance of the pharmacy environment and in the management and control of inventory
Enhances professional growth and development through participation in educational programs and in-services. BLS/PALS certification and ASHP Pediatric Certification
Performs quality assurance monitoring which includes:
Data collection, assessment, and evaluation
Problem identification, resolution, and follow-up
Report generation
Antimicrobial Stewardship Committee
Discharge and Admit medication reconciliation
Daily multidisciplinary rounding
Medication Safety Committee
Facilitates effective, efficient communication and problem-solving with inpatient and outpatient service providers. Uses Meditech and QS/1 to document activities, track performance outcomes, and produce performance reports.
Provide training and orientation, create reference materials, and share expertise with others.
Seeking continuing education opportunities, completing required in-services, and maintaining professional growth by attending various department, institutional, and external meetings, seminars, and workshops.
Applies performance improvement strategies to promote continuous process improvement.
Identifies opportunities and collaborates with the specialty pharmacy team/department to implement solutions. May serve on department—or hospital-wide committees as needed.
Collaborates with team, department, unit staff, and/or physician practice to meet expectations.
Organize monthly educational in-services, research and education
What we need from you:
Education: Pharm.D.
Related Work Experience: 1-2 year’s hospital pharmacy experience preferred.
Licensure/Certification: Clinical skills beyond traditional dispensing functions highly valued.
Current Illinois Registered Pharmacist License
Specialized Knowledge and Skills: Background in pediatric care desirable; should be physically able to get around in a fairly close environment; must be able to operate computers and general office type equipment
