La Rabida Children’s Hospital is hiring a full-time Patient Access Coordinator for the Registration, Scheduling, and Admissions department in Chicago, IL.

Job Summary:

The Referral Coordinator works to support excellence in patient care services by organizing the process for scheduling patient referrals that includes procedures and sub-specialty care consultation. The coordinator serves as a patient liaison and supports the scheduling of testing, procedures and appointments with outside facilities and institutions. The role works closely with physicians, administration, and staff to ensure that the facility is providing high quality care services to patients and their families. Referral Coordinator also collaborates with the Patient Centered Medical Home Administrator and ensures that outside facility care results are received and available in the patient’s medical record for primary care provider review.

Essential Patient Access Coordinator Functions:

Coordinate the process for scheduling patient referrals for sub-specialty care consultation appointments and procedures with other organizations and supports the scheduling of these appointments in the event of patient support is required.

Provides outside facilities and institutions with the appropriate paperwork to ensure that the referral appointment can be scheduled appropriately.

Confirms eligibility and requests authorization as needed per payer guidelines.

Maintains the master referral appointment list on shared drive to include outside facility patient appointment updates and ensures timely follow up on receipt of consult notes and results.

Responsible for ensuring that NCQA guidelines are adhered to in relation to referrals coordination and management.

Ensure adherence to HIPPA policies and procedures for all patients and actively protects all PHI.

Communicates to providers and staff changes occurring at outside facilities and institutions that may impact the timeliness or completion status of patient referrals, i.e., payer contracting changes, document/form changes, etc.

Conducts timely communication with physicians, case management and nursing staff to ensure timely submission of patient referral needs.

Performs other duties as assigned.

What we need from you:

Education: High School Diploma, some college preferred

Related Work Experience: 1-2yr clerical work experience required; Experience in health care and knowledge of medical terminology strongly preferred

Licensure/Certification: None

Specialized Knowledge and Skills: Demonstrated skills and expertise with Microsoft Office software preferred. Good verbal and written communication skills in the English language required.
