The Case Manager works as part of the client management team to facilitate all client engagement and case file maintenance, along with individualized treatment planning and implementation.Position Responsibilities:Case managementConduct program orientation, intake meetings, and develop an Individual Service and Goal Plan for new residentsProvide one-on-one case management services to residents, including advocacy, information and referral services (e.g., employment, housing, social services) (maximum caseload of 10 residential clients)Aide clients in establishing daily schedules and routines; help client develop other independent life skillsMaintain bi-weekly case notes and weekly progress notes as mandated by Guest House and the Virginia Department of Corrections (DOC) policiesMaintain case files and other required documentation according to agency and DOC policiesCommunicate regularly with relevant stakeholders as part of a resident’s treatment team (e.g., Probation Officers, Resident Specialists, etc.)Other duties as assignedGeneralEnforce compliance with Guest House rules: drug and alcohol policies, emergency procedures, etc.Maintain confidentiality at all times.Attend and participate in staff meetings and in-service training.Inform supervisor of client and facility needs and concerns.Work closely with management to avert or resolve problems related to both the clients and the facility.Licenses & CertificationsValid driver’s licenseCertified Substance Abuse Counselor Assistant (CSAC-A) (can be obtained on the job)CPR certification (can be obtained on the job)First Aid certification (can be obtained on the job)Note: Licenses/Certifications must remain in good standing/active during employment tenure with Friends of Guest House.QualificationsBachelor’s degree (master’s preferred) in social work, social services, criminal justice, or related field.Individuals with Virginia licensure (LPC, LCSW or LSATP) strongly desired, but not required.Five years of professional experience in a human services setting assisting at-risk populations with histories of addiction, mental illness, homelessness, domestic violence, severe trauma, etc.) is preferred.Excellent communication skills, including interpersonal, written, presentation and conflict management skillsExcellent organizational and time management skillsAbilities to work effectively with people of diverse backgrounds and exhibit a high level of cultural competenceProficiency at Microsoft officeReporting RelationshipsThe Case Managers report to the Director of Case Management.Job Type: Full-timeSchedule: alternative schedule available, hybrid schedule availableSalary Range: $50,000 – $55,000Full medical, dental, and vision insuranceFriends of Guest House (www.friendsofguesthouse.org) is a transitional program that supports women who are returning to the community after incarceration for non-violent offenses. Friends of Guest House provides a safe, caring, healing environment for women in recovery from trauma, addictions, challenged family systems, and mood disorders. We serve 30 women in our Residential Program at a time, and each woman is invited to participate in the program for 6 months and spend another 6-9 months in our Aftercare Program with stepped-down case management support. The goal of the women in the program is to re-enter the community, reunite with their families, and address barriers to achieving their goals. Overall, women who re-enter the community without support have a recidivism rate of 70%. Women who complete the Residential to Aftercare Program continuum at Friends of Guest House have a recidivism rate of 15%.
